Procalcitonin (PCT) is a type of chemical called a peptide precursor of the hormone calcitonin – a naturally appearing substance in the body involved in calcium homeostasis. This chemical element is composed of 116 aminoacids and is mainly produced by parafollicular cells (C cells) of the thyroid and by the neuroendocrine cells of the lung and the intestine.

The level of procalcitonin in the blood stream of healthy individuals is well below 0.25 ng/mL. Procalcitonin concentration significantly increases in a response to a proinflammatory stimulus, especially of bacterial origin, and its blood levels may rise to 100 ng/ml.

Procalcitonin is an ideal sepsis biomarker: it reflects the severity of bacterial infection and is in particular used to monitor progression of infection into sepsis, severe sepsis, or septic shock. The product concentrations in these potentially deadly medical conditions are typically above 1 ng/mL.

Non-septic infectious diseases like pneumonia, bacterial meningitis and malaria can also be diagnosed with the help of procalcitonin. Using procalcitonin measures and other diagnostic methods, doctors can more accurately prescribe treatments that, if not balance and customized for a patient, could end up being harmful.

A cluster randomized trial found that using serial procalcitonin-based algorithms to guide continuation or cessation of antibiotic therapy reduces the number of days patients are on antibiotics without adverse effects.

Procalcitonin may not be used as drugs, agricultural or pesticidal products, food additives or household chemicals.
